Log into your passport account.

Click on 'Results Entry & Live Scoring'.

Click on your clubs logo.

Find the fixture you wish to submit an injury report for and click 'Post Game'.

Click on the ‘Match Day Reports’ tab.

Click on the type of report you wish to submit. 

To report a serious injury complete the 'RFL Serious Injury Report'.

To report a head injury complete the 'Head Injury Form'.

Complete all the fields provided with as much detail as possible and then click on 'Submit'.
